<p>The Janus kinase-signal transducer and activator of transcription (JAK-STAT) signaling pathway is involved in cell immunity, division and death, as well as in tumor formation. The expression of key genes in the JAK-STAT signaling pathway in different types of cancer serves different roles. However, few reports are available on the prognostic value of the genes of the JAK-STAT signaling pathway in skin cutaneous melanoma (SKCM). The potential prognostic value of gene expression in the JAK-STAT signaling pathway in patients with SKCM was analyzed in the present study using data obtained from The Cancer Genome Atlas. To predict the potential functions and mechanisms of these genes in SKCM, gene set enrichment analysis (GSEA) and bioinformatics analysis were performed. A nomogram model including gene expression level and high risk factors was used to predict the risk level of prognostic. High expression levels of <italic>STAT1, STAT3, STAT4</italic> and <italic>STAT5B</italic>, and low expression levels of <italic>STAT6</italic> were associated with favorable prognosis [adjusted P&#x003C;0.001; hazard ratio (HR), 0.595; 95&#x0025; confidence interval (CI), 0.455&#x2013;0.778; adjusted P=0.018; HR, 0.725; 95&#x0025; CI, 0.555&#x2013;0.947; adjusted P&#x003C;0.001; HR, 0.590; 95&#x0025; CI, 0.450&#x2013;0.773; adjusted P=0.007; HR, 0.690; 95&#x0025; CI, 0.526&#x2013;0.940; and adjusted P=0.026; HR, 0.737, 95&#x0025; CI, 0.563&#x2013;0.964, respectively]. GSEA results demonstrated that these genes were involved in cell differentiation, invasion, adhesion, migration, cycle, colony formation and mitogen-activated protein kinase signaling. The combination of genes with favorable prognosis had a better effect on the overall survival (univariate survival analysis, P&#x003C;0.05). The results of the present study suggest that <italic>STAT1, STAT3, STAT4, STAT5B</italic> and <italic>STAT6</italic> gene expression may be used as a potential prognostic biomarker of SKCM, and the combined outcomes may exhibit a stronger interaction and higher survival time for SKCM.</p>

<title>Introduction</title>
<p>Skin cutaneous melanoma (SKCM) is a highly aggressive skin cancer, which arises from the malignant transformation of melanocytes in the basal layer of the epidermis (<xref rid="b1-ol-0-0-11287" ref-type="bibr">1</xref>,<xref rid="b2-ol-0-0-11287" ref-type="bibr">2</xref>) and has a poor prognosis, with a 5-year overall survival (OS) of 91.8&#x0025; worldwide (<xref rid="b3-ol-0-0-11287" ref-type="bibr">3</xref>). The number of new cases of SKCM that will emerge in 2019 was estimated to be 96,480, with the mortality estimated to be 7,230 (<xref rid="b4-ol-0-0-11287" ref-type="bibr">4</xref>,<xref rid="b5-ol-0-0-11287" ref-type="bibr">5</xref>). Therefore, there is an urgent need to identify novel biomarkers and prognostic predictive indicators for the detection and management of SKCM.</p>
<p>The JAK-STAT signaling pathway serves a crucial role in cell immunity, division and death, and in tumor formation (<xref rid="b6-ol-0-0-11287" ref-type="bibr">6</xref>). The two key components involved in this pathway are Janus kinases (JAKs) and signal transducer and activator of transcription proteins (STATs), which are encoded by the genes <italic>JAK</italic> (<italic>JAK1, JAK2, JAK3</italic> and <italic>TYK2</italic>) and <italic>STAT</italic> (<italic>STAT1, STAT2, STAT3, STAT4, STAT5A, STAT5B</italic> and <italic>STAT6</italic>), respectively (<xref rid="b6-ol-0-0-11287" ref-type="bibr">6</xref>).</p>
<p>A previous study has demonstrated that the key genes involved in the JAK-STAT signaling pathway are associated with several types of cancer, including breast (<xref rid="b7-ol-0-0-11287" ref-type="bibr">7</xref>,<xref rid="b8-ol-0-0-11287" ref-type="bibr">8</xref>), ovarian, lung, brain (<xref rid="b9-ol-0-0-11287" ref-type="bibr">9</xref>) and colorectal (<xref rid="b10-ol-0-0-11287" ref-type="bibr">10</xref>) cancer, and that their differential expression may result in different prognosis outcomes in different types of cancer. However, few reports about the association between these genes and SKCM are available, and further investigation analyzing the prognostic value of gene expression in the JAK-STAT signaling pathway in SKCM is needed. The aim of the present study was to identify the prognostic values of the expression of genes involved in the JAK-STAT signaling pathway in patients with SKCM based on data derived from public databases and bioinformatics analysis, and to explore the underlying mechanism that may affect the outcome in SKCM prognosis.</p>
</sec>
<sec sec-type="materials|methods">
<title>Materials and methods</title>
<sec>
<title/>
<sec>
<title>Data preparation</title>
<p>The Cancer Genome Atlas (TCGA; <uri xlink:href="https://cancergenome.nih.gov">https://cancergenome.nih.gov</uri>; accessed on September 10th 2018 was used to obtain the gene expression and clinical data of patients with SKCM, including sex, age and tumor stage. A total of 458 SKCM cases were selected after removing the cases with missing mRNA expression or clinical data and 0-day survival time.</p>
</sec>
<sec>
<title>Functional analysis of key genes in the JAK-STAT signaling pathway</title>
<p>The Kyoto Encyclopedia of Genes and Genomes (KEGG) signaling pathway map was generated using the KEGG website (<uri xlink:href="https://www.kegg.jp">https://www.kegg.jp</uri>; accessed on September 10th 2018 (<xref rid="b11-ol-0-0-11287" ref-type="bibr">11</xref>&#x2013;<xref rid="b13-ol-0-0-11287" ref-type="bibr">13</xref>). Gene ontology (GO) term analysis, including biological function (BP), molecular function (MF) and cellular component (CC), as well as KEGG enrichment analysis for JAK and STAT gene families, were performed using the Database for Annotation, Visualization and Integrated Discovery version 6.8 (DAVID; <uri xlink:href="https://david.ncifcrf.gov/tools.jsp">https://david.ncifcrf.gov/tools.jsp</uri>; accessed on September 13th 2018. The official gene symbol was used as the identifier; the species was <italic>Homo sapiens</italic> (<xref rid="b14-ol-0-0-11287" ref-type="bibr">14</xref>,<xref rid="b15-ol-0-0-11287" ref-type="bibr">15</xref>).</p>
</sec>
<sec>
<title>Gene interaction and association analysis</title>
<p>Gene-gene interaction analysis was performed using gene multiple association network integration algorithm (GeneMANIA; <uri xlink:href="http://genemania.org">http://genemania.org</uri>; accessed on September 15th 2018 using the default parameters (<xref rid="b16-ol-0-0-11287" ref-type="bibr">16</xref>,<xref rid="b17-ol-0-0-11287" ref-type="bibr">17</xref>). The correlation between JAK and STAT pathway gene expression in SKCM was evaluated using the Pearson&#x0027;s correlation coefficient; P&#x003C;0.01 was considered to indicate a significant correlation. Protein-protein interaction analysis was performed using the Search Tool for the Retrieval of Interacting Genes/Proteins (STRING; <uri xlink:href="https://string-db.org">https://string-db.org</uri>; accessed on September 25th 2018. The minimum required interaction score was 0.400 (<xref rid="b18-ol-0-0-11287" ref-type="bibr">18</xref>,<xref rid="b19-ol-0-0-11287" ref-type="bibr">19</xref>).</p>
</sec>
<sec>
<title>Patient grouping based on gene expression level</title>
<p>Patients with SKCM were divided into high or low expression groups using the median value, and the vertical scatter plots were generated.</p>
</sec>
<sec>
<title>Survival analysis</title>
<p>Kaplan-Meier survival plots with log-rank test were used to evaluate the OS for the high and low-expression groups of each gene and clinicopathological characteristics. In addition, the Cox proportional hazard regression model was used for univariate and multivariate survival analyses, and 95&#x0025; confidence intervals (CIs) and hazard ratios (HRs) were calculated. Joint effects analysis was performed for the combination of genes identified as significant by the survival analysis.</p>
</sec>
<sec>
<title>Nomogram</title>
<p>A nomogram was used to evaluate the association between JAK and STAT gene expression and clinical information in SKCM. In addition, the risk rank for each gene and clinicopathological characteristic, including age, sex and tumor stage, was evaluated by the risk points and total points. Survival rates (1-, 3- and 5-year) were also scored. A high score was associated with a low survival rate.</p>
</sec>
<sec>
<title>Gene set enrichment analysis (GSEA)</title>
<p>To investigate the potential underlying mechanism of the differential expression of <italic>STAT1, STAT3, STAT4, STAT5B</italic> and <italic>STAT6</italic> in SKCM, GSEA version 3.0 software (<uri xlink:href="http://software.broadinstitute.org/gsea/msigdb/index.jsp">http://software.broadinstitute.org/gsea/msigdb/index.jsp</uri>; accessed on September 20th 2018 (<xref rid="b20-ol-0-0-11287" ref-type="bibr">20</xref>) was used, and the difference in the expression levels in the low and high-expression groups for each gene was analyzed with reference gene sets, which were based on the Molecular Signatures Database sets c2 (KEGG gene sets, c2.all.v6.2.symbols.gmt), c5 [Gene Ontology (GO) gene sets, c5.all.v6.2.symbols.gmt] and c6 (oncogenic signatures gene sets, c6.all.v6.2.symbols.gmt) (<xref rid="b21-ol-0-0-11287" ref-type="bibr">21</xref>). The permutation number was set to 1,000. Those enrichment gene sets revealed by GSEA as exhibiting a nominal P&#x003C;0.05 and a false discovery rate (FDR) &#x003C;0.25 were considered to indicate a statistically significant difference. The default parameters were used in GSEA software.</p>
</sec>
<sec>
<title>Statistical analysis</title>
<p>Statistical analysis was performed using SPSS v.25.0 software (IBM Corp.) and R 3.3.5 (<uri xlink:href="https://www.r-project.org/">https://www.r-project.org/</uri>). Kaplan-Meier survival analysis and the log-rank test were used to calculate the OS and P-values for all associations. The Cox proportional hazards regression model was used for univariate and multivariate survival analyses. HRs and 95&#x0025; CIs were calculated using the Cox proportional hazards regression model with adjustment for influential clinical characteristics such as race, sex, age, Tumor-Node-Metastasis stage (<xref rid="b22-ol-0-0-11287" ref-type="bibr">22</xref>) and body mass index. FDRs in the GSEA were adjusted for multiple testing with the Benjamini-Hochberg procedure to control the FDR (<xref rid="b23-ol-0-0-11287" ref-type="bibr">23</xref>,<xref rid="b24-ol-0-0-11287" ref-type="bibr">24</xref>). P&#x003C;0.05 was considered to indicate a statistically significant difference. Vertical scatter plots and survival curves were generated in GraphPad Prism v.7.0 (GraphPad Software, Inc.).</p>

<sec sec-type="results">
<title>Results</title>
<sec>
<title/>
<sec>
<title>Clinicopathological characteristics of patients with SKCM</title>
<p>The clinicopathological data of the 458 patients included in the present study are presented in <xref rid="tI-ol-0-0-11287" ref-type="table">Table I</xref>. Race, age and tumor stage were significantly associated with median survival time (P=0.003, P&#x003C;0.001 and P&#x003C;0.001, respectively; <xref rid="tI-ol-0-0-11287" ref-type="table">Table I</xref>). After normalization, only <italic>JAK1, STAT1, STAT3, STAT4, STAT5B</italic> and <italic>STAT6</italic> genes mRNA expression were observed.</p>
</sec>
<sec>
<title>Bioinformatics analysis of JAK and STAT genes</title>
<p>The KEGG pathway map of the JAK-STAT signaling pathway is represented in <xref rid="f1-ol-0-0-11287" ref-type="fig">Fig. 1</xref> (KEGG map no. 04630; <uri xlink:href="https://www.genome.jp/dbget-bin/www_bget?pathway:map04630">http://www.genome.jp/dbget-bin/www_bget?pathway:map04630</uri>). The GO term and KEGG enrichment analysis for JAK and STAT genes included BP (<xref rid="SD1-ol-0-0-11287" ref-type="supplementary-material">Figs. S1A</xref> and <xref rid="SD1-ol-0-0-11287" ref-type="supplementary-material">S2A</xref>), CC (<xref rid="SD1-ol-0-0-11287" ref-type="supplementary-material">Figs. S1B</xref> and <xref rid="SD1-ol-0-0-11287" ref-type="supplementary-material">S2B</xref>), MF (<xref rid="SD1-ol-0-0-11287" ref-type="supplementary-material">Figs. S1C</xref> and <xref rid="SD1-ol-0-0-11287" ref-type="supplementary-material">S2C</xref>) and KEGG (<xref rid="SD1-ol-0-0-11287" ref-type="supplementary-material">Figs. S1D</xref> and <xref rid="SD1-ol-0-0-11287" ref-type="supplementary-material">S2D</xref>). The DAVID result for the combination of JAK and STAT gene families suggested that the JAK and STAT signaling pathways were associated with cell migration, the mitogen-activated protein kinase (MAPK) cascade, cell differentiation (<xref rid="f2-ol-0-0-11287" ref-type="fig">Fig. 2A</xref>), cytosol, cytoplasm (<xref rid="f2-ol-0-0-11287" ref-type="fig">Fig. 2B</xref>), DNA binding, ATP binding, signal transducer activity (<xref rid="f2-ol-0-0-11287" ref-type="fig">Fig. 2C</xref>), the PI3K-AKT signaling pathway and the pancreatic cancer signaling pathway (<xref rid="f2-ol-0-0-11287" ref-type="fig">Fig. 2D</xref>).</p>
</sec>
<sec>
<title>Gene interaction and association analysis</title>
<p>The gene-gene interaction network was analyzed separately in the <italic>JAK</italic> family (<xref rid="SD1-ol-0-0-11287" ref-type="supplementary-material">Fig. S3</xref>) and in the <italic>STAT</italic> family (<xref rid="SD1-ol-0-0-11287" ref-type="supplementary-material">Fig. S4</xref>), as well as in their combination (<xref rid="f3-ol-0-0-11287" ref-type="fig">Fig. 3A</xref>). The protein-protein interaction network is presented in <xref rid="f3-ol-0-0-11287" ref-type="fig">Fig. 3B</xref>.</p>
<p>Pearson&#x0027;s correlation coefficient was used to analyze the correlation between JAK and STAT genes in SKCM tissues based on the TCGA dataset (<xref rid="f3-ol-0-0-11287" ref-type="fig">Fig. 3C</xref>). The results suggested that, with the exception of <italic>STAT6</italic>, the expression of STAT genes (<italic>STAT1, STAT3, STAT4</italic> and <italic>STAT5B</italic>) in SKCM strongly correlated with each other.</p>
<p>Scatter plots of the expression of all JAK and STAT genes in TCGA dataset (using the median as the cut-off value) are presented in <xref rid="f3-ol-0-0-11287" ref-type="fig">Fig. 3D</xref>. The difference between the high and low-expression groups was statistically significant (P&#x003C;0.001).</p>
</sec>
<sec>
<title>Survival analysis</title>
<p>The results of univariate survival analysis of JAK and STAT genes are presented in <xref rid="f4-ol-0-0-11287" ref-type="fig">Fig. 4</xref> and <xref rid="tII-ol-0-0-11287" ref-type="table">Table II</xref>. High expression levels of <italic>STAT1, STAT3, STAT4</italic> and <italic>STAT5B</italic>, and low expression levels of <italic>STAT6</italic> were associated with a favorable prognosis (P&#x003C;0.05). Multivariate Cox proportional hazards regression analysis identified that sex, race, age and tumor stage were associated with the prognosis of patients with SKCM. Multivariate survival analysis, in agreement with univariate survival analysis, demonstrated that high expression of <italic>STAT1, STAT3, STAT4</italic> and <italic>STAT5B</italic>, and low expression of <italic>STAT6</italic> was associated with a favorable prognosis (adjusted P&#x003C;0.001; HR, 0.595; 95&#x0025; CI, 0.455&#x2013;0.778; adjusted P=0.018; HR, 0.725; 95&#x0025; CI, 0.555&#x2013;0.947; adjusted P&#x003C;0.001; HR, 0.590; 95&#x0025; CI, 0.450&#x2013;0.773; adjusted P=0.007; HR, 0.690; 95&#x0025; CI, 0.526&#x2013;0.940; and adjusted P=0.026; HR, 0.737; 95&#x0025; CI, 0.563&#x2013;0.964, respectively; <xref rid="tII-ol-0-0-11287" ref-type="table">Table II</xref>).</p>
</sec>
<sec>
<title>Predictive nomogram and joint effects analysis</title>
<p>Independent factors, including age, sex, tumor stage and mRNA expression, were integrated into the prognostic nomogram to predict the clinical outcomes of patients with SKCM (<xref rid="f5-ol-0-0-11287" ref-type="fig">Fig. 5A</xref>). Age, tumor stage, and <italic>JAK1, STAT1, STAT4, STAT5B</italic> and <italic>STAT6</italic> expression levels exhibited major contributions as prognostic signatures in the risk scores (range, 0&#x2013;100).</p>
<p>All survival related genes, including <italic>STAT1, STAT3, STAT4, STAT5B</italic> and <italic>STAT6</italic>, were selected and grouped to perform joint effects analysis. The grouping information of joint effects analysis is presented in <xref rid="tIII-ol-0-0-11287" ref-type="table">Tables III</xref>&#x2013;<xref rid="tVI-ol-0-0-11287" ref-type="table">VI</xref>. The group number with simple Arabic numerals represented all gene selected groups (<xref rid="tIII-ol-0-0-11287" ref-type="table">Table III</xref>); Arab numerals with brackets are 4 selected genes (<xref rid="tIV-ol-0-0-11287" ref-type="table">Table IV</xref>); Lowercase Roman numerals are 3 selected genes (<xref rid="tV-ol-0-0-11287" ref-type="table">Table V</xref>); Capitalized Roman numerals are 2 selected genes (<xref rid="tVI-ol-0-0-11287" ref-type="table">Table VI</xref>). The combination of genes with favorable prognosis had a better effect on the OS (univariate survival analysis; P&#x003C;0.05; <xref rid="tVII-ol-0-0-11287" ref-type="table">Table VII</xref> and <xref rid="f5-ol-0-0-11287" ref-type="fig">Figs. 5B-G</xref>, <xref rid="f6-ol-0-0-11287" ref-type="fig">6A-J</xref> and <xref rid="f7-ol-0-0-11287" ref-type="fig">7A-J</xref>).</p>
</sec>
<sec>
<title>GSEA</title>
<p>The detailed GSEA results, including KEGG, GO and oncogenic signatures, are shown in <xref rid="SD2-ol-0-0-11287" ref-type="supplementary-material">Tables SI</xref>, <xref rid="SD3-ol-0-0-11287" ref-type="supplementary-material">SII</xref> and <xref rid="SD4-ol-0-0-11287" ref-type="supplementary-material">SIII</xref>, respectively, and in <xref rid="f8-ol-0-0-11287" ref-type="fig">Fig. 8</xref>. High expression of <italic>STAT1</italic> was significantly associated with immune response (normalized P=0.002; FDR=0.243; <xref rid="f8-ol-0-0-11287" ref-type="fig">Fig. 8A</xref>), cell adhesion (normalized P=0.002; FDR=0.206; <xref rid="f8-ol-0-0-11287" ref-type="fig">Fig. 8B</xref>; normalized P=0.010; FDR=0.241; <xref rid="f8-ol-0-0-11287" ref-type="fig">Fig. 8D</xref>) and WNT protein binging (normalized P=0.006; FDR=0.261; <xref rid="f8-ol-0-0-11287" ref-type="fig">Fig. 8C</xref>). By contrast, low expression of <italic>STAT4</italic> was associated with WNT protein binding (normalized P=0.001; FDR=&#x2212;0.234; <xref rid="f8-ol-0-0-11287" ref-type="fig">Fig. 8E</xref>), whereas high expression of <italic>STAT5B</italic> was associated with gene silencing (normalized P=0.002; FDR=0.249; <xref rid="f8-ol-0-0-11287" ref-type="fig">Fig. 8F</xref>).</p>

<sec sec-type="discussion">
<title>Discussion</title>
<p>In the Surveillance, Epidemiology, and End Results Program database, the 5-year relative survival is 98&#x0025; for prostate cancer, 89.9&#x0025; for breast cancer and 19.4&#x0025; for lung cancer (<xref rid="b3-ol-0-0-11287" ref-type="bibr">3</xref>,<xref rid="b4-ol-0-0-11287" ref-type="bibr">4</xref>). For melanoma, a 91.8&#x0025; 5-year relative survival rate appears satisfactory; however, the 5-year relative survival for patients with tumor stage IV is only 3&#x0025; (<xref rid="b22-ol-0-0-11287" ref-type="bibr">22</xref>,<xref rid="b25-ol-0-0-11287" ref-type="bibr">25</xref>).</p>
<p>The JAK-STAT signaling pathway serves a crucial role in functions such as cell proliferation, differentiation, migration and apoptosis, cell survival in hematopoiesis, immune cell development, stem cell maintenance and organismal growth processes (<xref rid="b26-ol-0-0-11287" ref-type="bibr">26</xref>&#x2013;<xref rid="b28-ol-0-0-11287" ref-type="bibr">28</xref>). Dysfunction in the JAK-STAT signaling pathway is associated with diseases such as cancer and immune disorders (<xref rid="b6-ol-0-0-11287" ref-type="bibr">6</xref>,<xref rid="b27-ol-0-0-11287" ref-type="bibr">27</xref>). In the JAK-STAT signaling pathway, the <italic>JAK</italic> family comprises <italic>JAK1, JAK2, JAK3</italic> and <italic>TYK2</italic>, and the <italic>STAT</italic> family comprises <italic>STAT1, STAT2, STAT3, STAT4, STAT5A, STAT5B</italic> and <italic>STAT6</italic> (<xref rid="b6-ol-0-0-11287" ref-type="bibr">6</xref>,<xref rid="b29-ol-0-0-11287" ref-type="bibr">29</xref>). A number of diseases are associated with the expression levels of genes of the JAK-STAT signaling pathway. A previous study has demonstrated that increased expression of <italic>STAT3</italic> or <italic>STAT1</italic>, but not of both, was present in melanoma compared with that in benign nevi or skin melanocytes (<xref rid="b30-ol-0-0-11287" ref-type="bibr">30</xref>). In addition, active <italic>STAT3</italic> was present in melanoma (<xref rid="b30-ol-0-0-11287" ref-type="bibr">30</xref>). By contrast, overexpression of microRNA (miR)-214 in a lung cancer cell line reduced the expression of <italic>JAK1</italic>, which inhibited cell proliferation and colony formation, and suppressed cell migration and invasion (<xref rid="b31-ol-0-0-11287" ref-type="bibr">31</xref>). In papillary thyroid cancer, downregulation of <italic>JAK1</italic> by miR-520a-3p inactivated the JAK-STAT signaling pathway (<xref rid="b32-ol-0-0-11287" ref-type="bibr">32</xref>).</p>
<p>However, studies that explored the association between the prognosis of melanoma and the JAK-STAT signaling pathway are limited. In the present study, the expression of <italic>JAK</italic> and <italic>STAT</italic> family genes in melanoma was investigated based on TCGA data. The observation that JAK-STAT gene expression is associated with the MAPK signaling pathway is in agreement with a previous study, which demonstrated that the JAK-STAT signaling pathway is integrated with the MAPK signaling pathway (<xref rid="b33-ol-0-0-11287" ref-type="bibr">33</xref>&#x2013;<xref rid="b35-ol-0-0-11287" ref-type="bibr">35</xref>) and is associated with melanoma (<xref rid="b36-ol-0-0-11287" ref-type="bibr">36</xref>). Expression of <italic>JAK2, JAK3, TYK2, STAT2</italic> and <italic>STAT5B</italic> was not observed after normalizing mRNA expression in the present study, suggesting that these genes were expressed at a low level. High expression of <italic>STAT1, STAT3, STAT4</italic> and <italic>STAT5B</italic>, as well as low expression of <italic>STAT6</italic>, were associated with a favorable prognosis of patients with SKCM. These results are consistent with other studies, in which high expression of <italic>STAT1</italic> was associated with favorable prognosis in high-grade serous ovarian cancer (HGSC) (<xref rid="b37-ol-0-0-11287" ref-type="bibr">37</xref>), colorectal cancer (<xref rid="b38-ol-0-0-11287" ref-type="bibr">38</xref>) and esophageal squamous cell carcinoma (<xref rid="b39-ol-0-0-11287" ref-type="bibr">39</xref>). In addition, high expression of <italic>STAT1</italic> in HGSC was significantly associated with the recruitment of intraepithelial CD8<sup>&#x002B;</sup> T cells, which enhanced the prognostic and predictive value of intratumoral CD8<sup>&#x002B;</sup> T cells in HGSC (<xref rid="b40-ol-0-0-11287" ref-type="bibr">40</xref>), potentially due to tumors with high <italic>STAT1</italic> mRNA expression exhibiting elevated expression of genes specific for tumor-associated macrophages and immunosuppressive T lymphocytes (<xref rid="b7-ol-0-0-11287" ref-type="bibr">7</xref>). The results of the enrichment analysis in the present study also revealed that <italic>STAT1</italic> was associated with the immune response, which suggested that <italic>STAT1</italic> may accelerate the immune cell response to cancer (<xref rid="b41-ol-0-0-11287" ref-type="bibr">41</xref>). However, high <italic>STAT1</italic> expression was associated with poor prognosis in glioblastoma (<xref rid="b42-ol-0-0-11287" ref-type="bibr">42</xref>), and breast (<xref rid="b7-ol-0-0-11287" ref-type="bibr">7</xref>,<xref rid="b8-ol-0-0-11287" ref-type="bibr">8</xref>), ovarian, lung, blood and brain (<xref rid="b9-ol-0-0-11287" ref-type="bibr">9</xref>) cancer.</p>
<p>In contrast to that of <italic>STAT1, STAT3</italic> expression is downregulated in malignant pleural mesothelioma (<xref rid="b43-ol-0-0-11287" ref-type="bibr">43</xref>); however, the prognostic value of this association has not been reported. The majority of studies on <italic>STAT3</italic> and cancer prognosis have demonstrated that phosphorylated <italic>STAT3</italic> is associated with a poor outcome in colorectal cancer (<xref rid="b10-ol-0-0-11287" ref-type="bibr">10</xref>), multiple myeloma (<xref rid="b44-ol-0-0-11287" ref-type="bibr">44</xref>) and urothelial carcinoma (<xref rid="b45-ol-0-0-11287" ref-type="bibr">45</xref>). In addition, <italic>STAT4</italic> has been detected in several types of cancer, including prostate (<xref rid="b46-ol-0-0-11287" ref-type="bibr">46</xref>), breast (<xref rid="b47-ol-0-0-11287" ref-type="bibr">47</xref>), gastric (<xref rid="b48-ol-0-0-11287" ref-type="bibr">48</xref>) and ovarian (<xref rid="b49-ol-0-0-11287" ref-type="bibr">49</xref>) cancer. The upregulation of <italic>STAT4</italic> in hepatocellular carcinoma is associated with favorable prognosis, possibly due to the expression of <italic>STAT4</italic> in the immune cells; however, the function and mechanism of <italic>STAT4</italic> in non-immune cells remains unknown (<xref rid="b50-ol-0-0-11287" ref-type="bibr">50</xref>). High expression of <italic>STAT5B</italic> in astrocytoma is associated with poor prognosis (<xref rid="b51-ol-0-0-11287" ref-type="bibr">51</xref>), whereas high expression of <italic>STAT6</italic> is associated with poor prognosis in colon cancer (<xref rid="b52-ol-0-0-11287" ref-type="bibr">52</xref>).</p>
<p>The potential effects of these associations require further exploration. For instance, colorectal carcinoma cell lines exhibiting low <italic>STAT1</italic> and high <italic>STAT3</italic> expression levels are associated with enhanced tumor growth in xenografts; by contrast, xenograft cell lines with high <italic>STAT1</italic> and low <italic>STAT3</italic> levels grew slowly (<xref rid="b53-ol-0-0-11287" ref-type="bibr">53</xref>). Thus, gene interactions may influence the cancer outcome. The different expression level of these two genes in different types of cancer may serve different roles. Joint effects analysis in the present study suggested that any combination of the tested markers may have a higher prognostic value compared with that of any individual biomarker.</p>
<p>The STAT gene family affects cell differentiation (<xref rid="b54-ol-0-0-11287" ref-type="bibr">54</xref>), invasion (<xref rid="b55-ol-0-0-11287" ref-type="bibr">55</xref>&#x2013;<xref rid="b57-ol-0-0-11287" ref-type="bibr">57</xref>), adhesion (<xref rid="b58-ol-0-0-11287" ref-type="bibr">58</xref>) and migration (<xref rid="b59-ol-0-0-11287" ref-type="bibr">59</xref>), as well as cell cycle (<xref rid="b60-ol-0-0-11287" ref-type="bibr">60</xref>&#x2013;<xref rid="b62-ol-0-0-11287" ref-type="bibr">62</xref>) and colony formation (<xref rid="b39-ol-0-0-11287" ref-type="bibr">39</xref>,<xref rid="b63-ol-0-0-11287" ref-type="bibr">63</xref>) through the JAK-STAT signaling pathway; these processes are associated with the occurrence, development and outcome of cancer. The potential mechanism of how these genes affect prognosis should be further studied.</p>
<p>The present study investigated the prognostic value of the key genes in the JAK-STAT signaling pathway; only STAT genes were demonstrated to affect the prognosis of SKCM. In melanoma research, no golden standard exists for diagnosis or prognosis that would serve a similar role as the hairy-related 2 gene in breast cancer or prostate specific antigen in prostate cancer. Certain genes may act as biomarkers to predict the prognosis and mechanism of SKCM, including RAF (Raf proto-oncogene), MEK (MAP/ERK kinase), MAPK, RAS, myelocytomatosis oncogene and S100 calcium binding protein (<xref rid="b64-ol-0-0-11287" ref-type="bibr">64</xref>&#x2013;<xref rid="b69-ol-0-0-11287" ref-type="bibr">69</xref>). However, further research is required to identify a golden standard for predicting melanoma.</p>
<p>There were certain limitations in the present study. Firstly, only <italic>JAK</italic> expression data were reported following normalization; additional mRNA expression data are needed to further confirm these observations. Secondly, this is an association study. Further research is needed to explore the function and mechanism of the genes of the JAK-STAT signaling pathway identified in the present study in patients with SKCM. Another limitation of the current study is the limited sample size. Improved design and larger sample size studies are necessary to validate these results. Finally, there were no expression standards to measure whether the gene expression was high or low.</p>
<p>To the best of our knowledge, the present study is the first to evaluate the association between the expression of genes of the JAK-STAT signaling pathway and OS in patients with SKCM, and to identify the joint effects of prognostic values among the five identified genes. Overall, the results of the present study provided a novel insight into the function of these genes in SKCM clinical outcomes, and may be further utilized in the clinic for predicting the prognosis of SKCM.</p>
<p>In conclusion, the combination of the highly expressed <italic>STAT1, STAT3, STAT4</italic> and <italic>STAT5B</italic> genes, and the lowly expressed <italic>STAT6</italic> gene is associated with a favorable prognosis in patients with SKCM, and may be used as a novel biomarker for predicting the prognosis of patients with SKCM. The expression of the genes in the <italic>STAT</italic> family may affect the prognosis of SKCM by accelerating the immune response and immune cell activity, and by involving the MAPK signaling pathway. Further studies are required to validate these findings.</p>
