he oral cavity is increasingly recognised as a non-invasive window into systemic physiology. Saliva, gingival crevicular fluid, and oral microbiota harbour molecular signatures relevant to chronic kidney disease, diabetes mellitus, cardiovascular disease, neurodegenerative disorders, head-and-neck cancers, and periodontal–systemic comorbidities. Recent advances in electrochemical and optical biosensors, microfluidic lab-on-a-chip platforms, omics technologies, and artificial-intelligence-assisted pattern recognition are transforming oral fluids from supportive specimens into stand-alone diagnostic and prognostic tools, suitable for point-of-care and home-based monitoring. This Special Issue invites original research, systematic reviews, and short communications addressing the discovery, validation, and clinical translation of oral-derived biomarkers — including proteins, metabolites, microRNAs, extracellular vesicles, microbial signatures, and physicochemical parameters such as salivary pH and flow rate. We particularly encourage contributions on (i) novel biosensor platforms for chairside or wearable detection; (ii) machine-learning models integrating salivary, clinical, and imaging data; (iii) saliva-based monitoring of systemic chronic diseases and drug therapy; (iv) standardisation of pre-analytical workflows and regulatory considerations; and (v) the periodontal–systemic axis, including cardiometabolic, renal, and neuroinflammatory conditions. The aim is to consolidate evidence supporting saliva and oral fluids as reliable, scalable matrices for precision medicine, and to identify the methodological gaps that still hinder translation into routine practice. Submissions from clinical, translational, biomedical-engineering, and computational groups are welcomed, with the goal of producing a multidisciplinary reference resource for clinicians, biosensor developers, and digital-health researchers.
