Aortic stenosis - from pathophysiology to novel biomarkers and therapies

Calcific aortic stenosis is the most common valvular disease. Though older studies described aortic stenosis mainly as a degenerative disease, current reports underline its inflammatory background. Aortic stenosis is characterized by progressive inflammatory and fibrocalcific processes, oxidative stress, and neovascularisation in the valve and myocardial tissues. Currently, aortic valve replacement (AVR) and transcatheter aortic valve implantation (TAVI) are the only effective types of treatment in patients with severe aortic stenosis. No pharmacological treatment revealed efficient value in slowing or reversing the course of the disease. Optimizing medical and interventional therapy requires a more profound understanding of the pathophysiological background of aortic stenosis and the potential therapy modifications and modes.
